Well the idea is sleek as well as the unique.  A poisonous lipstick, where the snake is coming out of the lipstick mould rather than the expected lipstick color.

Take an image of a lipstick as shown below.

Duplicate the layer.  Using white as foreground colour and taking any solid  brush you can apply on the lipstick portion that is out of the mould.

Take an image of a snake.  You may have two scale it down in all scale it up so that it fits well on the mouth of the lipstick mould.

From bottom of the layer palette click on "Add vector mask". Press D to reset the foreground and background colors to default B&W. Select the brush tool. Apply apply the brush with black colour on the area of the snake that is outside the lipstick mould.

Doing this step will make this may look like coming out of the lipstick mould rather than being over it.

Using pen tool create a silhouette of the lipstick along with snake in a new layer and fill it with any solid colour.

From layer palette reduce the fill amount to 0%.

Double click the layer to open layer style window and apply settings as shown.

This level served as the drop shadow cast on the wall to create a simaulated realistic look.
